This form must be completed and returned to the Assistive Technology Coordinator, or other designated person, before technology from the Assistive Technology Lending Library will be dispersed.  Contact information is collected in case a problem is reported and service (by phone, email, or in person visit) is deemed necessary.  Please complete all fields as completely as possible.  If the borrower is under thirteen (13) years old, please have a guardian fill out this form.

I understand that this equipment has been loaned to me and is the sole property of Northeast Arc. I am expected to exercise due care in my use of this equipment and to utilize it only for the reasons it was borrowed. I understand and agree that:

All Northeast Arc issued equipment should be -

·         Used for the purposes loaned out

·         Stored and used carefully

·         Do not leave in car

·         Do not use while eating or drinking

·         Store/install in cool dry place

·         Keep away from pets

·         Do not allow other users

·         Attended to when used in any public area

·         Returned on time or be in contact with Assistive Technology Coordinator to extend loan time.

·         Reporting any Damage, loss, or theft of items immediately. Borrowers should not try to repair devices on their own but bring them back to the ARC to be serviced.

·         Used with the knowledge that any files on the device will be viewable to ARC staff (IT department and assistive tech coordinator) upon return.  All equipment memory will be wiped, and all files will be lost and no longer retrievable by borrower.

I have read, understand and agree to the above-mentioned terms governing the use of Northeast Arc’s issued equipment.

Policy or Standard: The Northeast Arc Assistive Technology Center receives some funding through The Department of Developmental Services (DDS) to purchase assistive technology equipment (devices, Laptops, tablets, apps, tools, utensils, etc.) to share with the broader community. This equipment is purchased with the intent to loan to interested parties for a trial period (fourteen days). Equipment is loaned with the understanding that it is the solely the property of the Northeast Arc and use is thereby limited to what is dictated by these policies. The loan is intended to be a trial period for the borrower to ascertain the degree to which the technology can support the individual in their daily life. They can then purchase the desired technology themselves or with funds the Northeast Arc can help them obtain. 

Equipment is reserved by contacting the Assistive Technology Coordinator. The Assistive Technology Coordinator will begin an assessment of the parties needs and make recommendations for the technology to be trialed. All required software should be preloaded onto the device before the loan is fulfilled. The trial period will last two weeks (14 days) and then the equipment is expected to be returned to the Assistive Technology Library (100 Independence Way, Suite N116E, Danvers MA 01923). Request for longer trials will be assessed on a case-by-case basis, and borrowers should contact the Assistive Technology Coordinator to make these arrangements.       

Procedure: 

Usage:  

Borrowers are required to comply with the following guidelines: 

• Equipment provided by the Northeast Arc Assistive Technology Library should be used primarily for purposes discussed between borrower and the Assistive Technology Coordinator. 

• Any damage, loss, or theft of equipment should be reported to the Assistive Technology Coordinator and all other relevant officials as soon as possible. Borrowers are prohibited from repairing or replacing hardware without consent of the Assistive Technology Coordinator. Damage should only be repaired by Northeast Arc staff and/or those persons designated by the Northeast Arc, unless otherwise decided by the assistive technology coordinator and Northeast Arc’s IT (Information Technology) department. 

• All software on devices is purchased and licensed by the Northeast Arc and does not own the copyright to this software or its related documentation. Borrowers are prohibited from installing or downloading software to Northeast Arc equipment without approval by the Assistive Technology Coordinator. Northeast Arc does not own the copyright to this software or its related documentation, and any deletion of the copyright notices in such software is prohibited. Borrowers may only use software, in accordance with the relevant software license agreement and are required to abide by any limitations contained in the applicable license, including limitations on reproduction and modification. Northeast Arc prohibits the illegal duplication of software and its related documentation. 

• Any data (saved passwords, WIFI networks, documents, photos, videos, etc.) stored on devices can be accessed by Northeast Arc employees upon return to the Assistive Technology Library. Furthermore, all equipment’s memory will also be wiped, and the device will be returned to factory settings. Therefore, borrowers should not expect to retain data stored on any Northeast Arc equipment. 

• The Northeast Arc’s equipment may not be used to communicate improper messages (e.g., messages that are threatening, vulgar, insulting, harassing, defamatory, derogatory, obscene, or otherwise inappropriate). Furthermore, Northeast Arc’s equipment, including but, not limited to the internet feature, is not to be used to access, view, download, send, copy, print or in any way communicate or view harassing or otherwise offensive material or messages, documents or files that include intimidating, hostile or offensive material based on race, color, religion, national origin, sex, age, disability, family status, sexual orientation, genetic information, or any other category protected by law. 

• Borrowers are prohibited from infringing on the rights of third parties, including intellectual property rights, whether relating to trademarks, copyrights, logos, multimedia creations, software, text, photos, or images of any kind. Any statement with respect to copyright may not be deleted; and reproduction, adaption, or modification of the work of an author without his or her consent is prohibited. Borrowers are prohibited from using links to websites which violate the law or infringe on third-party rights. 

 Instructions for Care: 

All Northeast Arc issued equipment -  

• Is to be stored carefully so it is not susceptible to damage.  

• Is not to be left unattended in any public area.  

• Is not to be left inside a vehicle where temperature extremes can permanently damage the unit and/or its components or could be visible resulting in theft.  

• Equipment cannot be transferred to another person; it is loaned to the borrower and the borrower is solely responsible for its care and return.  

• Equipment is to be returned to Northeast Arc after the fourteen-day trial period.  

Privacy: 

The borrower should have no expectation of privacy for the use of, or for the information contained in, Northeast Arc’s equipment. You should not consider any material on these systems to be private. Even erased or deleted material may remain available. To ensure that the use of electronic and telephonic communications systems and business equipment is consistent with Northeast Arc’s legitimate business interests, authorized representatives of Northeast Arc may monitor the use of its systems in their sole discretion, at any time, with or without notice, and notwithstanding any password code. Northeast Arc reserves the right to look at, listen to, or use anything on its systems, to by-pass any password code, and may conduct unannounced inspections of certain equipment. All personal data will be removed upon return of equipment to the Northeast Arc premises.
